Applicants are required to complete and submit the Gender-Based Violence and the Workplace Certification to certify that their organization has in place and has implemented a Gender-Based Violence and the Workplace Policy and has provided such policy to all of its employees, directors and board members. Provide a concise press-ready project description (500 characters or less) which identifies the grant recipient and other major project partners, provides a brief description of the project scope of work and describes the project goals/objectives.

Statement of need: Provide a brief summary of the need for the project in the geographic area proposed and the project's financing needs, including funding gaps of the proposed project.

Describe the community or area that is the subject of the proposed work. The description should be a brief, general overview of the place, focused on geography, character, population, economy, and environment.

Identify the current design or engineering status of the proposed project (e.g., conceptual, site plan, or no existing design). Specify the exact deliverables this grant funding will produce (e.g., Feasibility Study, Engineering Report, Preliminary Design, or 30% Design) and explain how these deliverables will meet the minimum requirements outlined in Section IV of the Request For Applications.

Explain how the project is located in an area identified as high-risk for coastal erosion, flooding, or other climate-related impacts affecting Lake Ontario's shoreline. Describe current or future hazards, and how those hazards may negatively impact the community.

Describe the anticipated resilience benefits and long-term outcomes of the proposed project including the reduction of hazard risk, protection of assets, ecosystem benefits, public benefits, and broader community benefits.

Describe how the proposed project is identified as a priority in existing planning efforts such as a comprehensive plan, hazard mitigation plan. Local Waterfront Revitalization Plan (LWRP), CLEAR plan, or climate adaptation plan.

Describe how the project design(s) will implement nature-based shoreline protection measures, resilient stormwater infrastructure, and/or blue-green infrastructure that directly benefit Lake Ontario coastal ecosystems and communities.

Describe how the project design incorporates and advances the principles of Article 14 of the NYS Environmental Conservation Law as outlined in the Request for Applications.

Describe how the project design will contribute to the overall goals of the CLEAR Initiative and enhance the resilience of coastal communities along Lake Ontario.

Describe how the project will incorporate the NYSDOS Resilience Principles and which principles the project incorporates.

Explain how the project will prioritize non-structural or nature-based approaches to promote resiliency.

Explain how completing the design, feasibility, or engineering work under this grant will reduce barriers to project implementation and make the project competitive for construction funding. Identify the specific future funding sources you intend to pursue (e.g., Clean Water, Clean Air, and Green Jobs Environmental Bond Act, Climate Smart Communities, federal grants, or local capital funds) and provide a realistic strategy and timeline for securing these funds once your 30% design is complete.

Describe the local capacity and administrative/organizational structure in place to advance and complete the project. Identify the individuals who will be responsible for specific tasks, such as contract and grants administration, fiscal accounting, and project and consultant management, and describe their relevant experience.

Describe your plan for public input and community engagement during the design process. Specifically, identify the members or roles of your proposed Project Committee, confirming that it will include at least one local resident who is not an elected official or municipal employee. Detail how the committee will advise on site selection and design reviews, and outline your strategy for broader public outreach (e.g., public meetings, surveys, or workshops).

Estimated Project Timeline: include project start/completion dates, estimates for design and planning including public outreach.

Describe and document how budget and costs were determined. Identify the person(s) responsible for compiling the budget; include relevant experience working on similar projects and the background of all parties. Include the method/approach that was used to arrive at estimates.

Describe how the project will be implemented using a complete scope of work. Identify the set of discrete tasks necessary to complete the project and for each task provide relevant deliverables, anticipated timeframe, key personnel or parties responsible for completion, and cost per task.
